Procedures for reporting whistleblowing are set out below: -

a. Complete the “Whistleblowing Form”.

b. Send the original “Whistleblowing Form” to one of the following contact points: -

1. For Board Members/ Customers/ Employees/ Agents/ Vendors/ Contractors/ Suppliers/ Consultants, complete the “Whistleblowing Form” and send your reply per Contact Point table.
2. For Senior Political Person Only, complete the “Whistleblowing Form” and send by By Post to 23-2, 2nd Floor, Jalan SL 1/10, Bandar Sungai Long, 43000 Kajang, Selangor Darul Ehsan, c/o: Company Secretary.
